Perfect Shot Trophy Guide – A Plague Tale Requiem
The Perfect Shot Trophy is one of the trophies that you can get early on in the game. In Chapter 1 – Under a New Sun, Hugo will ask Amicia to shoot the pine cones that he will send downstream.

This takes place just after the stealth tutorial.
To get the trophy, all you have to do is to shoot all the pine cones Hugo places in the small river. It’s as straightforward as that.
To have a clear view of the pine cones and to make your life much easier, stand just before the fallen log by the rocks. There, prepare to shoot at the pine cones by holding the R2 button and releasing it after a brief delay.
Aim at the pine cones as soon as Hugo sends them into the river so that you’d have plenty of time to shoot at them accurately. Take note that you can fire as many rocks at them as you can, assuming that you act fast enough.

Later on, another kid will come and join the fun. By this point, Hugo will send several pine cones in fast succession and the boy that joined in will throw rocks on the river to make shooting at them more challenging.

Just do your best to hit all the pine cones until a cutscene trigger where the kid gets a big rock that finishes the whole play session.
Say that you hit all the targets. The trophy should be awarded to you shortly. If not, just try this section of the game again.
